7. Mor Pati Association Temporary Home Commitment

By submitting this form electronically, I accept that I have read and understood it.

I know that approval of the application is not guaranteed; that the application may be rejected if false or incomplete information is provided or if the terms of the form are not complied with.

I accept that the information I provide in the form is accurate and that this information may be verified if deemed necessary.

I accept that I am fully responsible for the care and behavior of the animal I am temporarily hosting and for any situations they may cause, and that I will not hold Mor Pati Association liable in these matters.

I accept that Mor Pati Association may carry out reference checks during the application process and may request a home visit if deemed necessary.

If for any reason I become unable to care for the animal I am temporarily hosting, I agree to return the animal to Mor Pati Association and to give the association a reasonable amount of time to find a new home.
